Call for papers for: Journal of Integrated Care

Guest Editors: Prof Anne Hendry and Dr Helen Tucker

The special issue of the Journal of Integrated Care will focus on issues surrounding Transitional and Intermediate Care in order to provide safe, effective, timely and person centred care and support at home and closer to home.

We welcome submissions to this special issue. Topics covered include (but are not limited to):

  • Discharge to Assess pathways 
  • Creating a Single point of access
  • Reablement home care support
  • Hospital at Home and community acute frailty services
  • Technology enabled care solutions 
  • Professional to professional enhanced decision support for care homes
  • Person centred practice in community hospitals

  • Measuring experience, outcomes and quality of life following intermediate care
  • Case studies of international models of transitional and intermediate care
  • Financing intermediate care and reablement services
  • Caregiver education and support for self management at times of transition
  • Effective governance and financing of intermediate care
  • Practitioner competencies and training needs  
  • Identifying and managing delirium at home

Deadline and Submission Details
