It is globally recognised that a modern health care system needs to have strong primary care at its core. Alongside community based access, sufficient generalist and specialist clinicians and good support infrastructure, primary care requires successful integration between primary care professionals, acute services and social care resources. Primary care must also be more person centred and help people to draw on their own assets and those within local communities. This requires then a transformation in how primary care operates in which not only processes, pathways and structures are made integrated, but also our fundamental beliefs, values and paradigms.

This Special issue seeks to bring together knowledge and experiences from primary care clinicians, practitioners, leaders and researchers across the world.
Through research based, perspective pieces and practices case studies it will explore the following themes:

  • patient centric practice
  • inter-professional collaboration
  • new models of general practice and primary care
  • asset based approaches that connect with local communities
  • vertical integration between acute and primary care
  • work-based learning to support integration
  • achieving sustainable transformation
